Here is the downloadable application and what you need to do to send it to us.
Right Click on the links and select "save target as" for the 2012 application
2012 Side 2 Application (registration portion)
1. Download all 3 of the scans of the application, they will show up as photos. Open them and print them on plain white paper. (black ink is ok).
*Make sure you do not have the "fit picture to frame" box checked
2. Read the application completely and fill out the registration portion.
3. Mail (not email) us only the registration portion of the page along with everything listed on the "To Apply" section of the application.

*If you send us a CD or other such media with your photos on it, please include a self addressed and stamped envelope big enough to send it back to you. If you do not want it returned please make a note of that.
*If you know you are on our mailing list and want to download the application from this point forward please make note of that and we will take you off of our list.
